Our mission is to create an atmosphere in Fannin County where people of all ages are comfortable asking for behavioral health help and can easily find the resources that they need

Community Well TX began in 2019 as the Fannin Behavioral Health Leadership Team. In 2024, we adopted the new name to better reflect our widened scope, new team structure and expanded programs.

Improve the understanding of mental health and substance use disorders to encourage dialogue, compassion and treatment

Improve Resources

Improve access to behavioral health support and treatment, especially for those facing access to care barriers

Improve resilience through encouraging healthy mental habits and early identification and treatment of behavioral health issues with a focus on children

Fannin Walk 4 Hope

The Walk 4 Hope is held annually in Bonham, TX, and all funds benefit the Community Well TX, which sponsors and promotes mental health and suicide prevention programs in Fannin County. The next walk will be held on September 28, 2024. Follow the walk on Facebook to stay posted on the next event.

Baby Bright

Using the innovative Parents as Teachers curriculum, Community Well’s trained Parent Educators offer comprehensive home and community visits. We offer training, crafts and lesson plans tailored to the needs of each child, and we help to connect the famliy to community resources. Baby Bright is open to all residents of Fannin County with children ranging from prenatal to 5 years old.

Fannin Behavioral Health Leaderhip Team

The Fannin Behavioral Health Leadership Team (FBHLT) was formed in 2019 as a coalition of mental health professionals, non-profits, the criminal justice system, educators, the pastoral community, health care professionals and concerned community members. The FBHLT is a part of Community Well TX, and the team meets regularly to share, learn and collaborate to improve behavioral health solutions for Fannin County.

About Us

Community Well TX (CWTX) is a 501(c)(3) formed in 2019 by community leaders who share a desire to improve mental wellness for Fannin County residents.  We recognize that improving behavioral health outcomes is complex and that services are often fragmented. With this in mind, CWTX is broad-based coalition working together to improve direct behavioral health services in Fannin. Originally named the Fannin Behavioral Health Leadership Team, we adopted the Community Well TX name in 2024 to better capture our programs and mission.
